Le Fit Crew places great importance on protecting your personal information. This policy explains, in plain language, what information we collect, why, how we use and protect it, and what your rights are. It is established in accordance with Quebec's Act respecting the protection of personal information in the private sector, as amended by Law 25.

1. Who we are

Le Fit Crew (9532-8084 Québec Inc.) provides sports medical support services (sports therapy, injury follow-up) to teams, institutions and their athletes.

Our systems and data are hosted in Quebec, with a hosting provider bound by confidentiality.

3. Personal information we collect

3.1 Visitors to our website and people who contact us

  • When you book an appointment online, fill out a form or create an account: your name, contact details and the reason for your request.
  • Cookies strictly necessary for the operation of the site (see section 7).

3.2 Athletes and people who receive our medical support services

General information:

  • First and last name, date of birth;
  • Personal contact details;
  • Allergies and relevant medical conditions;
  • Emergency contact (parent or guardian): name, phone number, email address;
  • The last four digits of the health insurance number.

Health information (sensitive information):

  • Injury diagnoses and dates of incidents;
  • Clinical notes relating to injuries;
  • Parental consent, if the athlete is a minor.

Health information is sensitive information. We collect and use it only with your express consent and protect it with particular care (see section 5).

4. Why we collect this information

We use your personal information to:

  • provide and follow up on our sports medical support services;
  • communicate with you and manage appointments;
  • ensure athletes' safety (allergies, medical conditions, emergency contact);
  • bill our clients and, where applicable, process payments;
  • meet our legal and professional obligations.

We collect only the information necessary for these purposes.

5. Sensitive information and restricted access

Access to health information is limited to the people who need it in the performance of their duties, and only for the athletes they are actively supporting — not to all records in our database:

  • Medical staff (therapists and physicians): full access to the records of the athletes to whom they are actively providing services, according to their role within the teams concerned;
  • Coaches: access limited to name, diagnosis and injury-related notes, for the athletes on their teams.

Our systems are compartmentalized to enforce these access levels.

6. Consent

We collect and use your personal information with your consent, given freely and on an informed basis, for specific purposes.

  • Health information: your express consent is obtained by means of a consent form.
  • Minors: the consent of the parent or guardian is obtained.
  • You may withdraw your consent at any time (see section 12).

7. Cookies

Our website uses only strictly necessary cookies for its proper operation and the security of your session. We do not use advertising-tracking cookies, profiling, or third-party analytics tools.

You can configure your browser to block or delete cookies; however, some features of the site may then no longer work properly.

8. Disclosure to third parties and hosting

We do not sell or rent your personal information.

Your information is hosted in Quebec, with a hosting provider bound by confidentiality (management application, database and email service).

Outside Quebec: the only disclosure that may involve servers located outside Quebec is the processing of certain payments by the payment provider Stripe. This provider applies industry-recognized security measures. Athletes' health information is never transmitted to this provider.

We may also disclose information where required by law.

9. Automated processing and artificial intelligence

We make no decision based exclusively on automated processing of your personal information.

We may use artificial intelligence tools for internal and procedural purposes only. Under no circumstances are these tools used to process health information of our athletes, members or clients.

10. Retention period

We retain your information for the duration of our services, then for a standard retention period of at least five (5) years following the last service rendered, in alignment with the record-keeping obligations of the various professional orders governing our treatment professionals. At the end of this period, the information is anonymized or securely destroyed, anonymization being carried out according to the criteria provided by law.

11. Security measures

We take reasonable measures to protect your information, including:

  • minimizing the information collected — we collect only what is necessary for the effective provision of treatment and follow-ups;
  • hosting in Quebec;
  • role-based access control and compartmentalized systems;
  • technical and organizational measures appropriate to the sensitivity of the information.

12. Your rights

You have the right to:

  • Access — obtain a copy of the information we hold about you;
  • Rectification — have inaccurate, incomplete or equivocal information corrected;
  • Withdrawal of consent — withdraw your consent at any time. We will then stop using your information for the provision of our services and destroy or anonymize it, except for information we are required to retain under our applicable legal or professional obligations, or under our retention period (see section 10);
  • Portability — obtain the computerized information that you provided to us, in a structured, commonly used technological format;
  • De-indexing — request that the dissemination of information cease or that a hyperlink be de-indexed, in the cases provided by law.

13. Confidentiality incidents

In the event of a confidentiality incident (for example, unauthorized access to or disclosure of personal information) presenting a risk of serious injury, we take the necessary measures to limit its consequences, we notify the persons concerned and the Commission d'accès à l'information, and we record the incident in a register.

15. Changes to this policy

We may amend this policy from time to time. Any change is published on this page, together with the update date. We invite you to review it regularly.

This policy is provided in French and in English. In the event of any discrepancy, the French version prevails.
